When your first appointment is confirmed you will receive an invitation to the patient portal with a link to intake forms.  Please fill these out at least one week before your appointment* so the doctor has time to review them as well as any recent or relevant lab work or imaging. 
(*Please note, if intakes are not received by the deadline, Doctor Anne reserves the right to cancel your appointment)
Lab work or imaging can be uploaded as a document in the patient portal or faxed to 631-282-8910 by you or your doctor.

NOTE: due to the need to be able to collect money virtually, we need to ask patients to enter a credit card number** to reserve an appointment. That credit card will never be charged without you being notified,  
We require a 24 working hour notice for cancellations (by previous Thursday if your appointment is Monday);
under 24 hours is a 25% charge;  a no show is billed 50% of their appointment
cancellation fees are not covered by insurance

*Telemedicine patients seeing Dr. Van Couvering must be within the boundaries of Vermont, New Hampshire or Massachusetts during their visit.
